Consent of the legal guardian
You received paperwork regarding one of your clients who is in the palliative care unit. The paperwork is regarding the treatment options the resident requires immediately. When you reviewed the paperwork, you identified the legal guardian of the client has not signed the paperwork.
- Can you use the paperwork without legal guardian’s signature? Why or why not?

Facilitating individual service planning and delivery
Ms Mona has been living at a large residential aged care centre for several months, when her long-term partner, Ms Julie tells the manager she is very worried that Ms Mona is becoming increasingly distant, withdrawn and depressed. Ms Mona avoids mixing with the other residents and shows little interest in the outings her partner organises. Ms. Julie is Ms. Mona’s nominated carer and partner. Ms Julie is concerned and speaks to the manager of the service about this.
The reason for Ms Mona agitation becomes clear when a nurse hears a volunteer helper insisting Ms Mona to go along with her family’s wish for staying with her family at their home. The family is against Ms. Mona’s relationship with Ms. Julie.
Ms Mona is a diabetic patient with high cholesterol levels and high blood pressure. The family believes Ms Mona is mentally unfit and requires some medical treatments. The aged care facility has analysed the situation and prepared a service plan with the assistance of Ms Julie to look after Ms. Mona.
Ms. Julie is really concerned after knowing what happened and discussed the situation with Ms Mona and aged care facility that they are planning to move the care facility in next 30 days.
The aged care home is committed to treating everyone with dignity and respect. The home develops an awareness and training program with the clear goal of making sure that all staff, including volunteers, have the understanding, skills and motivation to deliver services that are LGBTI responsive, inclusive and sensitive. It arranges a meeting with the family to explain that Ms Mona’s identity is not negotiable and discuss how they can work together to support her. The home reviews its policy to reflect that the client is the person who nominates who they want involved in their care.

Duty of Care
Rex is 70 years old male living in the Rosedale care facility. Rex has stroke about 5 years ago which left him with left sided weakness. Rex was very active and socially interactive person before having stroke. He loves to participate in all the activities of the facility and he goes out every alternate Friday to RSL club to have catch up with his friends. Rex could go independently before having a stroke.
After having stroke, Rex condition does not permit him to go out of the facility independently. Facility management decided has provided carer to accompany Rex every Friday. Rex now has electric scooter which he uses to go to the RSL club. Carers have reported that Rex does not listen and drinks a lot. Sometimes Rex gets quite intoxicated and on one occasion, the worker had to assist Rex to throw up when he returned from the club.

Strength based approach
Tim is 55 years old male who had motor vehicle accident about five years ago. He had lost strength of his lower limbs. He uses wheelchair to get around the house. Tim’s parents are very helpful towards his care activities. Tim was a fully qualified professional health care trainer before the accident.
This accident has led Tim to give up his profession, but Tim is still very healthy and determined to educate people about healthy choices. Tim has a meeting with his case manager (Rebecca) today and he expressed his wish to continue his profession to educate older people to maintain healthy choices. Rebecca was fully satisfied after meeting Tim about his career choice. She is happy to get Tim enrolled in the course to update his qualification up to the current industry requirements’. Tim has to attend the TAFE to update his qualification. Rebecca is quite positive that Tim can still fulfil his profession and bring positive change in older people. Tim’s parents are very concerned about Tim’s ability to study again because of his age and injuries.
