Angela Cortez is a 17-year-old female who presents to the clinic c/o of fatigue, decreased energy and interest in pleasurable activities. She reports having stress at school as a high school senior and fears not getting into college because of difficulty concentrating in her classes and as a result her grades have declined. She reports that her boyfriend broke up with her a few months ago for her best friend; she has been crying every day since and has gained 25-30 lbs weight since. She denies suicidal ideation but wishes that she could disappear from everyone and everything. Her physical exam showed no sign of self-harm. Mild, comedonal acne, acanthosis nigricans at the nape of her neck, and stribciae on her abdomen and buttocks.
