HIRARC Preparation for Construction Site
Answer the following questions in report format and combine the report content into single PDF document for submission.
Scenario:
You have been appointed as the Site Safety and Health Officer (SHO) for a commercial building development project currently in the pre-construction stage. The project involves the construction of a multi-storey office complex incorporating underground works, reinforced concrete structures, and steel framework installations.
Before site activities commence, the project director has instructed you to establish a comprehensive HIRARC to identify potential safety and health risks associated with the construction phase. This document will function as a primary safety planning tool and will be referenced throughout the project duration.
The HIRARC must be prepared in accordance with the Department of Occupational Safety and Health (DOSH) Malaysia – Guidelines for Hazard Identification, Risk Assessment and Risk Control, or other recognized international safety standards.
Task Requirements:
1`. Project Overview:
Provide a concise overview of the proposed construction project, including:
• Type and purpose of the building
• General location and surrounding environment
• Overall project complexity and safety challenges
2`. Hazard Identification:
Identify three (3) major construction activities expected during the construction phase. For each activity, identify three (3) potential hazards
Hazards may fall under, but are not limited to:
• Physical hazards (e.g. machinery, falls, moving vehicles)
• Chemical hazards (e.g. dust, fumes, cement exposure)
• Ergonomic hazards (e.g. manual handling, awkward posture)
• Psychosocial hazards (e.g. fatigue, work pressure, stress)
List each hazard clearly under the relevant work activity.
3. Risk Assessment:
For each identified hazard, assess the level of risk. Use a risk matrix to categorize the severity and likelihood of each hazard. The typical scale for risk assessment is:
• Likelihood
• Consequence
• Risk Rating: A combination of likelihood and consequence
The risk assessment shall reflect realistic site conditions and typical construction practices.
3. Risk Control Measures:
Propose appropriate control measures for each hazard by applying the Hierarchy of Controls, in the following order:
1. Elimination
2. Substitution
3. Engineering controls
4. Administrative controls
5. Personal Protective Equipment (PPE)
Each control measure must be practical, relevant to construction work, and aligned with legal safety requirements.
Submission Requirements:
1. Detailed Report Format
• Provide an introduction to the project and the importance of HIRARC for safety.
• For each activity, clearly explain each hazard identified, the reason behind the risk rating, and the rationale for selected control measures.
• Conclude with a summary of the overall risk level for each activity after control measures are applied and any further recommendations.
• Research and Reference: Support your analysis with relevant standards and guidelines, such as OSHA (Occupational Safety and Health Administration), local safety regulations, or construction safety best practices.

The criteria of report submission should fulfil the followings: – • Using font Arial, size 11, spacing 1.5 and Justified paragraph.
