Rather than the real-world scenarios of the Discussion Application assignment, this Integration assignment provides more practice connecting the same concepts to an actual aggregate or population in your CHN’s community. The information you present in this assignment will then also be used for the Written Integration and Discussion Application assignments in Weeks 7 & 8. Consider the work a component towards completing your Weeks 7 & 8 assignments.– Keep the information you gather for future use.

Steps

State the identified marginalized (vulnerable)aggregate or population.
Review the aggregate or population within the context of vulnerability with detail – describe the vulnerabilities from concepts and resources we explored in the weeks 4 & 5 (there are likely more than one) of your identified aggregate or population. For example, think about the various health, social and biological determinants. Describe at least 3 vulnerabilities of the vulnerable aggregate or population with detail, based upon the health, social, environmental and biological determinants of health.
Discuss how each of the 5 social determinants apply to the identified marginalized aggregate or population.
Gather current data (you many need to use a variety of sources, and you may need to go back and interview your CHN for information as well) that will illuminate the impact of social determinants upon the identified marginalized aggregate or population. For example, if your vulnerable population is LGBTQ+ youth and young adults, some evidence might be the rate of mental illness, suicide, employment, homelessness, other health risks, etc. These data point to the presence and depth of the vulnerability.
Acting as a CHN, choose a social determinant for which you would like to focus upon to support resilience within the vulnerable aggregate or population in the community. Explain why you chose that determinant.
Research and then describe 1 collaborative community program idea and 2 related intervention ideas that will help you reach your goal of supporting resilience and health within the chosen aggregate or population. One intervention idea should be directly related to nursing, and 1 intervention idea should be community collaboration-based
Discuss how the interventions can be adapted for use for your CHN’s specific practice setting.
Write a conclusion that summarizes the challenges of the identified vulnerable population, the social determinants that most impact upon its resiliency, and your thoughts about the proposed community program and interventions.
