a Quality Improvement project to resolve or correct a patient safety issue by
following the Model for Improvement steps.
· Introduction: Describe a patient safety issue that exists within an
organization and explain how that clinical issue impacts provider practice
and/or population health
· Develop two (2) Aims statements that address the
issue.
· Identify at least four (4) pertinent team
members including description of team roles and rationale for selection.
· Establish one (1) Outcome Measure with rationale
for selecting the Outcome.
· Identify one (1) Change Concept appropriate for
this issue and identify a specific change that can be
tested using PDSA. Use the Model for Improvement
(MFI) framework.
· Select one (1) implementation strategy for the
tested change that will promote spreading the improvement.
