· PMH: HTN, osteoarthritis, insomnia, BPH

· Medications: Hydrochlorothiazide, diphenhydramine nightly, tamsulosin, ibuprofen PRN

· Reports dizziness and forgetfulness

Vitals:

· Orthostatic BP positive

· MMSE: borderline impairment
