Written Evaluation PlanAbout the programI have worked with the parent, teacher, and youth mediation program for four years. This program aims to ensure a good connection between familiesparticularly an effective communication strategy between parents and children in enhancing better modes of communicating efficiently. For instance, we ensure that we establish crucial positive partnerships with the parents of teenagers to build understanding and trust; as a result, it leads to united working towards supporting the children’s wellbeing and development. We impact different skills through such a program, such as enhancing active listing skills as a foundation of communication, speaking to parents in a transparent, respectful, and considerate way, addressing concerns with a problem-solving approach, keeping a positive attitude about working together, and keeping the expectations of youth realistic about what can be done at home. Therefore, the top objective of the parent, teacher, and youth mediation program is to ensure that it leads to happy family life through practical communication tips.Besides, the program is founded on the foundation of serving clients. For instance, it has enrolled a majority of the persons-250 customers over the period where 150 clients compose parents, whereas 100 of them are the adolescents. There is a clear guideline in observing the enrollment takes which occurs on the turnout progress and the connectivity ability. For example, families may be self-referred or referred by a previous client or by the school social workers or instructors. Primarily, the referral program is the most efficient method as it leads to positive changes in the child, and the parent-adolescent relationship improves. On the secondary level is when a family or a teacher refers a family to our program to participate and experience the positive aftermath changes of the youth at the learning center.On the one hand, our referral programs are structured systematically. For instance, the referred clients take an hour into the initiation intake completed with the school social worker. Additionally, preliminary data is entered while demographic and short assessment of information is required on the child-parent relationship scale. Not only do the Parent, Teacher, and Youth Mediation Program take a required course of nine weeks, but also, weekly, the social worker and teacher help in the facilitation of the session. On the other hand, the program adequately stakes time in examining Child-Parent-Relationship through the CPRS. This is administered at every start of the activity with one’s family members. However, in these routines, teachers get involved throughout the session as the learner develops a student-teacher relationship while at school; however, it is limited. Therefore, the program sufficiently alleviates the challenges faced by both parties through problem-solving approaches employed.Evaluation questionsBased on the prompt, I am proposing a comprehensive evaluation to combine the processes and the outcome results. Through the evaluability assessment trend, I will ensure that there is a proper evaluation of events of the child and the parent before assessing of extension of services. The initial stage ensures adequate information as there is sufficient analysis of the goals, needs, and objectives of the family throughout the initiative. Similarly, the evaluation assessment stage will help save time as there will be an examination of critical elements of clients, and they provide unrealistic information or are unwilling to pursue change. Besides, the assessment will aid in the determination of bringing light on the client’s underlying issues, and proper steps may be undertaken to make future evaluation feasible. Therefore, this method is essential as it is crucial in examining the client’s information which assists in decision making.Data Collection and Sampling
