Cognitive Behavioral Therapy (CBT) is a widely used, evidence-based psychotherapy designed to address maladaptive thought patterns and behaviors across individual, group, and family settings, each offering distinct therapeutic advantages and implementation challenges. Individual CBT offers very individualized care, giving Psychiatric Mental Health Nurse Practitioners (PMHNPs) a chance to customize the interventions to the needs of individual clients and develop a strong therapeutic relationship and thorough analysis of emotions (Beck, 2020). Ebrahimi et al. (2024) prove the ability of individual CBT to decrease cognitive distortions and rumination among adolescents with social anxiety disorder, which proves the ability to provide a targeted intervention. Nevertheless, individual therapy has several drawbacks, such as a slower rate of treatment and a lack of access to social learning.

Group CBT is based on individual therapeutic aspects, such as peer support, universality, and modeling, in which members learn through the collective experience and acquire the skills to cope in a social context. Whereas group settings are cost-effective and do provide normalized experiences, PMHNPs struggle to cope with group dynamics, balance fair participation, and avoid dominant members monopolizing the discussion. Also, not all members feel comfortable discussing personal difficulties in front of an audience, and comparison of members would inevitably normalize maladaptive behaviors or lead to a competition that is not conducive to therapeutic education.

Family CBT deals with relational aspects, which affect the mental state of a specific individual and bring systemic change based on the enhancement of communication and boundary settlement (Samar et al., 2023). Studies conducted by Le Grange et al. (2020) prove that effective CBT with family-based treatment is effective in treating adolescents with eating disorders, and this factor underscores the importance of combining both individual and family-oriented treatment. Nonetheless, PMHNPs face severe issues in the family environment, such as working around intra-familial complexity, avoiding emotional bargaining, and tackling collusion of the family members who may manipulate the therapy to maintain control instead of the change. It would be crucial to create clear boundaries and make all voices count to achieve therapeutic success.

To address these issues in any of the settings, the PMHNPs are advised to use systematic protocols and remain flexible, encourage free communication, set clear ground rules, and make all participants feel respected and involved. By adapting CBT principles to each setting’s unique demands, therapists maximize treatment effectiveness while addressing the diverse needs of individuals, groups, and families.

For this Discussion, as you compare the use of CBT in individual, group, and family settings, consider challenges of using this approach with groups you may lead, as well as strategies for overcoming those challenges.
