Module 4: Group Project: Social Determinants of Health in Our County Instructions:
As a team, research the social determinants of health (SDOH) in your county of residence using the County Health Rankings website: https://www.countyhealthrankings.org/Links to an external site.. Your goal is to analyze key health factors, compare your county to the state (or a neighboring county), and identify priority areas for intervention.
Group Tasks:
Research & Data Collection: Gather data on social determinants of health, including income levels, education, healthcare access, environment, and more. Comparison Analysis: Compare your county’s health indicators to state averages or a neighboring county. Priority Issue Identification: As a team, discuss and determine the most critical social determinant that Advanced Practice Registered Nurses (APRNs) should address in your county. Intervention Strategies: Propose solutions or interventions that APRNs can implement to improve community health outcomes. Group Deliverables:
Collaborative Report (X pages): A structured document summarizing your research, comparisons, and proposed interventions. Presentation (e.g., PowerPoint, Infographic, or Video): Visualizing your findings and recommendations. Discussion Engagement: Each group will review and respond to the presentations of at least two other teams, providing constructive feedback and insights. Contribute a minimum of 5-6 pages. It should include at least 3 academic sources, formatted and cited in APA.
